Patient onboarding forms are essential tools in dental fields for efficiently gathering critical information from new patients. These forms help streamline the intake process, ensuring that all necessary data is collected to provide optimal care and manage patient records effectively. Recreating your Onboarding Forms in CareStack allows for a paperless experience, providing both users and patients with the convenience of signing and maintaining records electronically.

To create a patient onboarding form, perform the steps below.
- Navigate to the System Menu > select Practice Settings > Office Wizard.
To Create an Onboarding Form:
- Click + Start from scratch.
- Use the Search Box to search for existing Forms or Letters.
Create a new Form or Letter modal will appear. Enter the details as needed:
- Workflow Type: Select whether this document is a Form or a Letter.
- Name: Enter the name of your onboarding form.
- Document Type: Select whether this is a Patient Consent Form, Financial Disclosure Form, or any other document type.
- Location: This document will only populate for new patients added to the specified locations.
- Add to Clinical Chart: Select this option to add this form or letter to the clinical chart.
- Add to Patient Onboarding Form: Select this option to have this form automatically populate for each new patient added to the locations selected in the field below and choose Trigger for All patients or Trigger for patients with Age Above/Below/Between/Equals for the selected age.
- Click Create.
- Click Save as Draft if you want to come back and fill in this later.
Create the body of your intended form by using the following elements.
Click +Add Header to input a header into the form.
- Select the Logo Alignment: Choose Left, Center or Right.
- Choose whether to use your Account-level, Brand-level, or Location-level information for the Header Logo and Header Address. Users can also opt for None.
Click and drag Form Components to fill the form with different types of elements such as Heading, Short Answer, Paragraph, etc.
- Click and drag Presets from the right side of the screen to enter a placeholder field to be auto-populated.
- Build the form and fill in the elements as needed.
To manage a System Component.
- Drag and drop the intended System Component into the form and customize as needed.
- Rename field headings, if required.
- Use Make it Mandatory on applicable components to ensure completion before signing.
Click +Add Signature to add signatures to the form.
- Patient Signature: Select this option to require a patient signature on the form.
- Provider Signature: Select this option to require a provider signature to complete the form.
- Witness Signature: Select this option to add a witness signature on the form.
- Click Save as Draft at the bottom-right of the screen when you are finished.
- To publish the form, click Publish.
- This form will now automatically populate for new patients that are created in the selected locations.
