CareStack features a dedicated perio chart specifically designed for recording periodontal factors affecting patient's oral health. Within this chart, you can document and monitor measurements for pocket depth, gingival margin, mobility, bleeding, and other parameters. This comprehensive approach enables a thorough assessment of the patient's periodontal health, facilitating tailored treatment plans.

Let's add a Perio Chart and explore its key features.
Select the patient using the Global Search bar > select Clinical > Perio Chart.

Once in the Perio Chart module, you will notice five tabs that will let you view and work with your periodontal data in different ways.
- You will enter your measurements on the Perio Chart tab, with or without a graph.
- Add Bleeding and Plaque indicators directly to the Perio Chart.
- You can enter and focus on numerical measurements on the Numeric Chart tab.
- Compare exams in the Perio Graph tab and the Numeric Comparison tab.

- When the Perio Chart page loads and no periodontal exams are recorded, it will display 'No prior Perio Examination'.
- Click +Add Chart to add a new perio chart.
- Select the Provider and Location.
- The exam date will be automatically selected.
- You can change the provider and location during an ongoing examination.

Note: By default, the priority is as follows:

- To add a post-dated perio chart, checkmark the Select a previous Date checkbox and select the Exam Date.
- Click Add.

Before inputting the measurements, the providers / dental assistants can take a look at the patient’s clinical history by clicking on the Clinical Summary on the top right.
- This will open up the clinical slide-out which shows the Patient’s latest added Prescriptions, Lab Cases, Allergies and Conditions, Medications if any, and Medical History Forms.
- Click on the 'i' icon to view information about Abbreviations and Symbols.
- To add a note, click Add Note which opens the Notes modal.
- Enter the number of your measurements in the blue-highlighted box. The chart may auto-advance to the next field to help you complete the chart faster.
- To chart a measurement of 10 or deeper, hold the ALT key and enter the full number.
- To show special indicators on a pocket:
- Type B to show Bleeding.
- Type S to show Suppuration.

Tip: If a measurement of 10+ is required, Hold the ALT key then type the full number (10, 11, etc.). To enter a negative gingival margin, Hold the ALT key then type the number.

Click on the 'Show Lingual Teeth' toggle button, initially disabled by default, to switch between a concise visual display of all values in one frame and a graphical representation of the lingual layer.
| Note: The selected toggle option for Show Lingual Teeth remains unchanged during user login or logout, retaining the user's preference. |
- You will see highlighted red markings for readings that surpass the threshold. We have also categorised the teeth with distinct lines to facilitate a swift correlation between the individual tooth and their respective readings for effortless identification. These have been implemented to provide users with an enhanced Perio Charting experience.
- Utilise the right-hand side panel for effortless mapping of conditions or to input data points as required. This eliminates the need to switch between the Numeric and Perio Chart.
To ensure clarity, extracted implants will now display with an ‘X’ over the Implant draw type, allowing users to clearly see that an implant was present for a specific tooth and has since been extracted.
- If an extraction (existing or completed) is recorded after an implant, the implant extraction will be reflected in the Perio Chart.
- When both implant and extraction are marked as existing, the implant is prioritised. For existing and completed treatments, the completed treatment takes priority.
Once you have finished charting your perio exam, click Finish.
A toast notification will appear on the top right stating 'Perio chart saved successfully.'
- The next time a user tries to add another Perio chart, the latest Perio chart will be displayed by default, along with the Exam date, Provider associated with the charting, and the Location at the top.
- The Exam dropdown will list the previous exams in descending order.
- You can switch between the various exams using the exam drop-down.
- Now when a new Perio chart is added, there is an additional checkbox 'Copy from the previous exam'.
- By default, it is unchecked.
- On checking the box, previous measurements will be copied to the chart overriding the existing measurements, if any.
- If you need to edit the Provider and the Location after completing a perio examination, click Edit.
Click View Statistics to display categorised summaries of pocket depth, gingival margin, furcations, mobility, and bleeding on probing based on the highest recorded values per tooth.
- Click the Print icon to print the measurements after completing the Perio Chart, as needed.
