Filling out a medical history form in CareStack is an important step in ensuring accurate patient care and effective communication between healthcare providers. When you enter your medical history into CareStack, you provide vital information that helps healthcare professionals understand your health background, current conditions, and any medications you may be taking. Filling out a Medical History Form can be done with just a few clicks.

Let’s get started!
- Select the patient using the Global Search Bar.
- Click on Med. Hx.
- Click + Add Form to open a form template.
- You need to fill up medical history information for Conditions, Allergies, Current Medications, Dental Questionnaire, and Medical Questionnaire.
- Mark Yes, No, or I don't know for each condition or allergy.
- You can see some conditions or allergies grouped as a category.
- To mark all conditions or allergies in a category as No, click Mark as No on the category header.
- To add a note, click + Note and enter the required details.
- If none of the items are applicable to the patient, you can skip all items in the Conditions or Allergies tab by clicking Mark all as No.
- Once a section is completed, click Next.
In the Current Medications tab,
- You can add new medications or edit current medications prescribed for the patient.
- To end a current medication, click End Medication.
- You can also Edit the Start Date and End Date of the existing medication by clicking on the pencil icon.
- Once you have entered or edited the Start Date and/or End Date, click Save.
To Add Medication:
- If the patient has no existing medication and medications were recorded previously, the Date when the last medication was added will be displayed in this section.
- To add a new medication, click + Add Medication.
- Enter the Medication Name, Start Date, and End Date.
- Click Add to add the medication to the patient's medical history form.
- Once added, click Next.
After covering all the sections, you can complete the form by:
- Adding Patient and Provider Signature: Click Add Signature under the corresponding headings (Patient and Provider) and add the signature. OR,
- Click Print & Sign to complete the form.
| Note: You can save the form as a draft by clicking Save as Draft. This will allow you to save the current progress and resume the form later. |
- On the resulting pop-up, click Proceed to confirm.
- Once the form is completed, you can access it from the left-hand side.
- The line item will display the date when the form was completed.
- You cannot edit a medical history form once it is completed.
- To add a note, click + Note and enter the required information as needed.
- Click Print Blank Form to take a printout of the medical history form without the responses.
| Note: The conditions, allergies, and current conditions marked for a patient in a completed medical history form will be displayed in the patient's Medical Alerts. Hover over the medical alert to view who added the information and when it was added. |
