- Select the patient using the Global Search Bar > click Med. Hx.
OR
- Search for the patient using the Global Search Bar.
- Click on the Forms quick link.
- Select the required medical history form.
- Click on the Print button on the far right.
Note: An additional option is that you can print blank forms so that you can directly fill out the physical copy without any hassle. To do that, click Print Blank Form. |
